What you need to know
Your MyShyft profile must be completed and approved before you can begin working shifts. Completing your profile accurately helps prevent onboarding delays and allows team to verify your information faster.
Required documents and forms may vary depending on:
- Your state
- Your license or certification
- The type of care setting you work in
To speed up the approval process – especially if you are signing up over a weekend – you can begin uploading and completing the items below as soon as possible. Visit our FAQ page to view document and forms necessary.
Steps to Follow
1. Download the MyShyft app – create your account and log in using your email and phone number.
2. Complete Your Personal Information – make sure the following information is accurate:
- Full legal name
- Contact information
- Date of Birth
Your information should match your government-issued identification and professional license records.
3. Upload Required Documents
You may be asked to upload some of the following:
- Government Issued ID
- TB Test
4. Complete Required State Forms
Complete the professional screening form for your state.

Ohio: Fill out form here
5. Add Personal Information
You will also need:
- Email addresses for two professional references*
- Professional license information
*Can be added directly in the app
6. Monitor Your Profile Status
After submitting your information:
- Check the app regularly for updates
- Respond quickly if additional documents are requested
- Make sure uploaded files are clear and readable
Incomplete or blurry uploads may delay approval
Common Mistakes to Avoid:
- Uploading expired documents
- Submitting blurry or cropped photos
- Entering a name that does not match your ID or credentials
- Forgetting to complete state-specific forms
- Missing professional reference information
