About the Opportunity
MyShyft is partnering with a hospice facility in the Tampa Bay area to provide Licensed Practical Nurses for Continuous Care and Crisis Care, primarily in patients’ homes, with additional opportunities to support inpatient coordination.
This is an ongoing opportunity — not a short-term assignment — available on a full-time or PRN basis for an LPN who wants consistent access to shifts. Nurses work independently in the field while receiving strong support from the broader care team, and pay is set by the nurse and processed as soon as a shift is clocked out.
This role requires prior hospice experience and comfort managing higher-acuity patients independently in patient homes, along with willingness to travel within Pinellas County.
Position Overview
- Provide Continuous Care (CC) and Crisis Care for hospice patients primarily in home settings, with opportunities to support inpatient coordination.
- Deliver extended, hands-on bedside care to high-acuity hospice patients.
- Work 12-hour shifts — days, nights, or extended-hour cases — on a full-time or PRN basis.
- Operate independently in the field while staying connected to and supported by the broader hospice care team.
- Travel within Pinellas County between home visits and any inpatient coordination needs; reliable personal transportation required.
Key Responsibilities
- Provide Continuous Care to hospice patients experiencing acute symptom management or end-of-life crisis needs.
- Deliver extended bedside care for high-acuity patients in the home, with support for inpatient coordination as needed.
- Monitor patient status closely and communicate changes to the hospice care team promptly.
- Administer medications and treatments within LPN scope of practice, per physician orders and facility protocols.
- Support patients and families through symptom management, comfort care, and emotional support during crisis periods.
- Complete timely, accurate documentation in accordance with hospice and regulatory requirements.
- Work independently while coordinating with the interdisciplinary hospice team as needed.
Qualifications
- Active, unrestricted Licensed Practical Nurse (LPN) license in the state of Florida.
- At least 18 years of age.
- Hospice experience required; Continuous Care experience preferred.
- Comfortable working independently in patients’ homes.
- Ability to manage higher-acuity patients with confidence and sound clinical judgment.
- Willingness to travel within Pinellas County; reliable transportation required.
