About the Opportunity
MyShyft is partnering with a hospice agency in the Franklin, Tennessee area to provide Licensed Practical Nurses for Continuous Care and Crisis Care shifts, supporting high-acuity patients in both home and facility settings.
This is an ongoing opportunity — not a short-term assignment — for an LPN who wants consistent access to shifts without a long-term commitment. Nurses work independently in the field while receiving strong support from the broader care team, and pay is set by the nurse and processed as soon as a shift is clocked out.
PRN, 1099 independent contractor; shift-by-shift flexibility; no long-term commitment required
This role requires prior hospice experience and comfort managing higher-acuity patients independently in patient homes.
Position Overview
- Provide Continuous Care (CC) and Crisis Care for hospice patients across home and facility settings in the Franklin/Nashville area.
- Deliver extended, hands-on bedside care to high-acuity hospice patients.
- Work primarily day shifts (8–12 hours), with some night shifts available, on a PRN basis.
- Operate independently in the field while staying connected to and supported by the broader hospice care team.
- Travel within the Franklin/Nashville area between home and facility visits; reliable personal transportation required.
Key Responsibilities
- Provide Continuous Care to hospice patients experiencing acute symptom management or end-of-life crisis needs.
- Deliver extended bedside care for high-acuity patients in home and facility settings.
- Monitor patient status closely and communicate changes to the hospice care team promptly.
- Administer medications and treatments within LPN scope of practice, per physician orders and agency protocols.
- Support patients and families through symptom management, comfort care, and emotional support during crisis periods.
- Complete timely, accurate documentation in accordance with hospice and regulatory requirements.
- Work independently while coordinating with the interdisciplinary hospice team as needed.
Qualifications
- Active, unrestricted Licensed Practical Nurse (LPN) license in the state of Tennessee.
- At least 18 years of age.
- Minimum 1 year of hospice experience required; Continuous Care experience preferred.
- Comfortable working independently in patients’ homes as well as facility settings.
- Ability to manage higher-acuity patients with confidence and sound clinical judgment.
- Willingness to travel within the Franklin/Nashville area; reliable transportation required.
