1

Identify the Right Setting for Your Goal

Your first decision is choosing which type of facility you want to work in. Adult foster care homes, homes for the aged, and community mental health group homes all allow medication-related tasks by trained non-licensed staff under Michigan rules. Hospitals and skilled nursing facilities do not. This choice determines everything about your training path and scope of work.

AFC · HFA · CMH group homes = medication tasks allowed
2

Apply to a LARA-Licensed or MDHHS-Authorized Facility

Adult foster care and homes for the aged facilities are licensed by LARA’s Bureau of Community and Health Systems, while community mental health programs are overseen by MDHHS. Confirm the facility is licensed and in good standing before accepting a position, unlicensed operations cannot legally delegate medication tasks to direct care staff.

Facility must be LARA-licensed or MDHHS-authorized
3

Complete the Facility’s Medication Management Training

Your employer delivers the training, not the state. Many HFA and AFC facilities use the Health Care Association of Michigan (HCAM) Medication Management Program, which offers both an Instructor Program (training facility trainers) and a Student Program (directly training direct care staff). The training covers oral medications, topical treatments, eye and ear drops, medication schedules, MAR documentation, side effect monitoring, and safety protocols specific to the facility type.

Employer-delivered · HCAM program widely used in HFAs and AFC
4

Demonstrate Competency Under Licensed Nurse Supervision

Before you administer medications independently, you must demonstrate competency under the supervision of a licensed nurse (RN or LPN) at the facility. This is a facility-managed evaluation, there is no third-party exam. The licensed nurse confirms you can safely follow the care plan, document correctly, and identify and report side effects.

Competency verified by licensed nurse · facility-managed evaluation

Work Within Your Facility’s Scope and Ongoing Supervision

Once trained and competency-verified, you can administer medications within your specific facility under the supervision structure required by MDHHS or LARA rules. This authority is tied to your current employer and setting, it does not transfer to a different facility type, and it does not make you a Medication Aide in any portable or certified sense.

01

Assuming Michigan Offers a Standard Med Aide License

It doesn’t. There is no MDHHS-issued Medication Aide credential. Anyone telling you otherwise is mistaken.

02

Enrolling in Out-of-State QMA or CMT Programs

Indiana’s QMA, Illinois’s CMT, and Ohio’s MA-C are not recognized in Michigan and will not provide medication authority here.

03

Assuming AFC Training Transfers to a New Employer

It doesn’t. Your medication authority is employer-specific. Every AFC or HFA must train and verify you independently.

04

Confusing CNA Scope with Medication Roles

CNAs cannot administer medications in SNFs or hospitals in Michigan. That requires nursing licensure, not a CNA credential.

05

Working Without Verifying Facility Licensing

Only LARA-licensed AFCs and HFAs can legally delegate medication tasks to non-licensed staff. Working for an unlicensed facility creates legal risk.

06

Performing Tasks Beyond Your Training

IV medications, injections, and nursing assessments remain outside the scope of non-licensed staff regardless of experience level.

Demand Factor Context
Aging Population Michigan has over 1.7 million residents aged 65+, and this population continues to grow each year.
Home Care Preference MI Choice Waiver and MDHHS Home Help programs support increasing demand for home-based care services.
Direct Care Wage Increases MDHHS Home Help individual caregiver rate increased to $17.13/hr effective January 1, 2026.
SNF and AFC Growth Michigan has approximately 440 licensed skilled nursing facilities and thousands of Adult Foster Care homes statewide.
LPN / RN Shortage Nursing shortages in Michigan mean facilities increasingly rely on well-trained non-licensed staff in eligible care settings.
Turnover Challenge High direct care worker turnover creates steady job openings, especially in AFC and group home environments.
